Please select from the forms below. You can download the form by clicking on the appropriate button for the form you need. There is also option to download all the forms at once if you need to fill them all out. Please check your downloads folder.
Once you have downloaded the form(s), do one of the following:
- Review and sign the form electronically using a digital signature or a Fill and Sign option. You must have Adobe Acrobat to use this option. Once completed, email the forms to counselingcenter@ihccinc.com.
- Print out the forms. Sign them and then scan them to your computer and email them to counselingcenter@ihccinc.com.
- Print out the forms, fill them out and bring them with you to your next appointment.
